Intake/Assessment Coordinator
Aiken Regional Medical Centers, located in Aiken, South Carolina, is a 273-bed acute care facility providing quality healthcare to the residents of Aiken and surrounding communities. Aiken Regional Medical Centers has been ranked a top hospital in South Carolina by the Carolina Center for Medical Excellence for its treatment of heart attack, heart failure and pneumonia. Services provided at the hospital include emergency medical care, orthopedic surgeries, maternity, and behavioral health services.

Assessment Coordinator
Responsible for conducting mental health assessments, communicating with psychiatrists in determining level of care needed, facilitating admissions and referrals to and from other agencies, conducting pre-certifications and utilization reviews, and fostering positive relationships with all referral sources.
Essential Job Duties:
-Addresses needs of callers seeking assistance.
-Conducts mental health assessments.
-Facilitates referrals to and from other treatment providers as appropriate.
-Communicates with referral sources to ensure satisfaction and coordination of care.
-Performs pre-certifications in a timely manner.
-Performs utilization reviews in a timely manner.
-Communicates with physicians and hospital staff regarding utilization review issues.
-Educates staff on the correct procedure for answering Respond calls.
-Keeps units and Emergency Room stocked with forms necessary to complete tasks.
-Maintains updated and detailed Tracking Log.
-Prepares monthly statistical report on assessments.
-Communicates feedback from referral sources to Management Team.
-Inputs assessment data into the appropriate computer programs upon completion of each assessment.
-During periods of critical staffing, will assist clinical staff in patient rounds, help ensure pt safety and comfort, and assist with non-clinical patient care duties under the supervision of a Mental Health Technician, LPN or RN.
Requirements include:
Education requirements: Registered Nurse or minimum of a master’s level in a human services field. Full licensure preferred.
Work experience requirements: Minimum one year experience in mental health setting.
