Job description
Social Work Assistant Responsibilities
- Intake Assessment (Center tour, documentation, eligibility check and follow up as needed.)
- Individual and group counseling to elderly participants
- Provide customer care services which might include answering or making phone calls on behalf of participants and/or family, assist with PCP and other appointments, identifying and resolving immediate needs and problems, linking clients to community resources (i.e. IHSS, ACCESS, etc.)
- Conduct Home Evaluations as needed
- Complete assessments, progress notes, flow sheets, and quarterly reports in a timely manner.
- Assist with opening and maintaining participants charts
- Coordination of participant discharge planning
- Other duties as assigned
- Must be bilingual in Chinese (Mandarin and/or Cantonese)
